Accreditation Area Profile

In NetForum, an accreditation application is also an accreditation area. The Accreditation Area Profile allows you to view and modify information about one organization's application for accreditation.

Accreditation Area Profile Information

The Accreditation Area Profile displays information in the following sections:

• Organization Information
• Accreditation Information
• Survey Contact Information

Organization Information

Field	Description
organization	Displays the name of the organization seeking accreditation. Click the linked organization name to go to the Organization Profile.
org type	Displays the type of organization.
date founded	Displays the date the organization was founded.
address info	Displays the address of the organization.
phone	Displays the organization's phone number.
fax	Displays the organization's fax number.
e-mail	Displays the organization's email address.
ceo	Displays the name of the organization's chief executive officer.
cmo	Displays the name of the organization's chief medical officer.
cao	Displays the name of the organization's chief administrative officer.

Accreditation Information

Field	Description
code	Displays the accreditation program code.
status	Displays the accreditation program status.
name	Displays the accreditation program description.
type	Displays the accreditation program type.
start date	Displays the date that the accreditation program begins.
end date	Displays the date that the accreditation program ends.
decision date	Displays the accreditation program decision date.
decision letter sent to	Displays the name of the contact in the organization to whom the decision letter for the accreditation program was sent.

Survey Contact Information

Field	Description
name	Displays the name of the survey contact.
phone	Displays the phone number of the survey contact.
e-mail	Displays the email address of the survey contact.
fax	Displays the fax number of the survey contact.

Accreditation Area Profile Actions

You can perform the following actions from the Accreditation Area Profile:

• Edit the Profile
• Add an Affiliated Program (add branches)
• Add a Survey

Accreditation Area Child Forms

The child forms section of the Accreditation Area Profile is divided into three tabs. Each tab contains a child form with information related to the accreditation area or application.

The following tabs are available:

• Accounting
• Organization
• Survey

Accounting Tab

Use the Accounting tab to view invoices relating to this accreditation for this organization. The Accounting tab has the following child form:

• invoices

Organization Tab

Use the Organization tab to view organizations and add branches, or affiliated programs. The Organization tab has the following child form:

Survey Tab

Use the Survey Tab to add and view survey details. The Survey tab has the following child form:
